You can submit a permission form online through our website! Be sure to follow our rules below before submitting your form!
- Permission form must be submitted at least 2 days before the permission date
- After submitting the permission form, student must contact the HR department immediately
- Each permission form must be approved by the HR department before it can be use
- The permission form can be denied by the HR department if the reason behind it is not qualified
- Those who have their permission form denied and still insist to go, will receive at least one warning letter
